Temporary Handicap Parking Permit application forms can be obtained from the Chief's Secretary at Police Headquarters or visit http://www.state.nj.us/mvc/pdf/Vehicles/HDC_Placard_Temp.pdf and bring the completed form to the Chief's Secretary during normal business hours. The Chief's Secretary is available from 8:30 am to 4:00 pm, Monday through Friday (except holidays).
Requesting individuals must complete the application and have their doctor complete the appropriate portion.
Completed application forms should be returned to the Chief's Secretary along with a check or money order in the amount of $4.00 payable to: NJ Motor Vehicle Commission. CASH WILL NOT BE ACCEPTED.
Temporary Handicap placards are good for a period of six (6) months from the date-of-issue. They may be renewed for an additional six (6) months but only one (1) time. Please contact the Chief's Secretary for renewal instructions at (908) 753-1149.
Permanent Handicap Placards are provided by the NJ Motor Vehicle Commission ONLY. Forms for such a request must be obtained from the Motor Vehicle Commission directly.
